Truck-driver document needs / Montgomery and Hope Hull

Broken trailer seal?
Need an affidavit notarized?

Bring your company-provided paperwork or an eligible statement you prepared in your own words. Gillstar Group can discuss a mobile appointment to perform the appropriate notarization.

A practical appointment when you are on the road.

When a commercial trailer seal is broken or compromised, the shipping or receiving facility may ask the driver to complete an affidavit or written incident statement explaining what happened. Facilities may provide their own paperwork. The driver completes the statement in their own words.

Gillstar can discuss mobile appointments at approved trucking facilities, warehouses, truck stops and distribution centers in Montgomery and Hope Hull. Other nearby Central Alabama locations can be discussed. Facility permission, an appropriate meeting point and access must be confirmed.

Gillstar is typically available 24 hours a day, including evenings, weekends and overnight appointments, subject to scheduling and confirmation. After-hours appointments depend on availability. Contact us before waiting at or traveling to a meeting point.

Bring your paperwork or a prepared statement.

If the facility or company provides a form, bring it along with any instructions. Complete your explanation in your own words. If no company form is available, you may prepare your own statement. Gillstar may notarize an eligible prepared statement using the appropriate notarial certificate.

  • Tell us your city or general meeting location, number of signers and appointment window.
  • Have your paperwork and recipient instructions available.
  • Ask about acceptable identification and when the statement should be signed.
  • Keep load, customer, identification and other confidential details out of the initial general inquiry.

We do not provide company-approved incident forms, draft legal affidavits or choose the facts or wording of your explanation. Ask the requesting party to clarify its requirements.

Your statement. Our notarial role.

The driver is responsible for the explanation in their statement. Gillstar performs the appropriate notarization. We do not investigate the incident, determine responsibility, certify that the explanation is true, inspect cargo or provide legal advice.

Notarization does not guarantee that a shipping or receiving facility will accept a particular document, release a load or resolve a dispute. Gillstar is independent and does not imply affiliation with a carrier, shipper or receiving facility.
